Most people spend more time researching a car purchase than their Medicare plan. These five decisions — made during a 7-month window that only comes once — determine your healthcare costs for years.

Advantage vs. Supplement — Getting It Wrong Is Expensive

Medicare Advantage looks cheaper upfront — zero premium sounds hard to beat. But out-of-pocket maximums can reach $8,850/year, networks restrict your doctors, and prior authorizations delay care. A Medigap Supplement with a slightly higher premium often costs far less when you actually need care.

The right answer depends entirely on your health, your doctors, and how often you use care.

The Enrollment Window: Missing It Has a Permanent Penalty

Your Initial Enrollment Period (IEP) is a 7-month window surrounding your 65th birthday. Miss it without qualifying employer coverage, and you face a 10% permanent late enrollment penalty on Part B — for every year you were late — added to your premium for life.

A 2-year delay = 20% permanent premium surcharge. Many retirees carry this penalty for 20+ years.

Part D Drug Coverage: Skipping It Costs You Twice

Many healthy retirees skip Part D drug coverage because they don't take prescriptions now. That's a mistake. Like Part B, Part D carries a late enrollment penalty — 1% of the national base premium per month you were without coverage. Add one new prescription in year 3, and suddenly you owe back-penalties on top of the premium.

When you first enroll in Part B, you have a guaranteed-issue right to any Medigap Supplement — no health questions, no medical underwriting. Wait even 6 months, and that window closes. Afterward, insurers can deny you or charge more based on health. Switching from Advantage to Supplement at 70 after a health event is often impossible.
